New Patient Forms

No one likes long waiting times at the doctor's office. So, to speed the process along, we ask that you download the forms below, fill them out at home, and bring them with you to your first appointment. This eliminates much time in the waiting room and allows us to spend more time with you in person.

Step one: Download the pdf file of each form (total of 3).
Step two: Fill them out.
Step three: Remember to bring them with you to Dr. Garrison's office.

Most people have this already installed on their computers, but if you do not, you may download it for free. (It's easy and fast, and having this will make it easier for you to view lots of other files in the future, so don't be shy!)

 

 

 

A word about the pre-surgical process

The Garrison Center is here to help you not only with the medical aspects of losing weight, but with ALL the issues required for your success. Many patients need assistance during the pre-surgical process of  obtaining clearances from physician specialists and authorizations from their insurance companies.

A special team of experts at the Garrison Center is dedicated to facilitating this process for you. Whereas it commonly takes several months to complete all the pre-surgical steps at other centers, patients at the Garrison Center can expect just three weeks between their initial consultation and authorization for surgery.
